The Official Open Diary Survivor2 Application
Please take the time to read over the eligibility rules before taking the time to fill this Application out. If you applied for the previous Survivor but were unsuccessful, you will be required to fill this application out if you want to try again. This application has changed since the last Application was released so please fill this the new one out. This Application should not take more than 30 minutes to complete. Please read it over and proofread it carefully before emailing it to us. If you'd like to take your time to fill it out, you have 50 Days to submit it. The deadline for submission is September 30th, 2001 at Midnight EST (changed from Sept. 25th, 2001). Please copy the Application below, fill it out, and email it to us when you have completed it. The Final Stage
As you know, along with this Application comes many things you are required to agree to. Successful Applicants will be notified by e-mail in mid to late-October. Further details will be posted on the OD Survivor Diary and Website as they become available.
This Application must be submitted by September 25th at Midnight EST.

As as OD Survivor II Applicant, I have read and agree to the Eligibility Rules provided.
I agree to stay committed to OD Survivor for as long as I'm not voted off regardless of personal issues. (Unless they are out of my control).
I understand, that if caught cheating or violating any of the OD Survivor Rules throughout the course of the Game, I will be disqualified without prejudice.
I applied for OD Survivor I but was unsuccessful.

OD Survivor has the right to disqualify any individual if caught cheating or violating any of the OD Survivor Rules (including, if the individual does not meet one or more of the eligibility rules).
