Story: Princess Mileena and her twin sister Kitana were the daughters of Shao Kahn, leader of the Outworld. Or at least, that's what the public was led to believe.
After the takeover of a civilized world called Edenia, Shao Kahn took the queen, Sindel, as his wife and the crown princess, Kitana, as his daughter. As she reached adulthood, Kahn realized what a danger she could become, and had Mileena, a deformed clone, created to make sure Kitana's loyalty stood fast.
To make sure she kept a close eye on her "sister," Mileena trained as an assassin alongside the princess, they both soon rose to great heights and became Shao Kahn's personal assassins. Mileena acquired a pair of magical sai that flamed on command, and could shoot flaming replicas of themselves at her foes. She quickly became a deadly weapon in Kahn's arsenal.
In 1994, the Earth Warriors came to the Outworld to face off with Kahn himself. When Kitana entered, Mileena immediately suspected her of ulterior motives other than to serve Kahn, and entered herself to make sure her sister didn't get out of hand. It was at the tournament that she met Baraka.
Baraka had been sent to destroy the Shaolin Temples before the second tournament. He shared her deformity: abnormally large, and sharp, teeth that could bite through flesh or bone. They quickly became lovers. The tournament passed uneventfully and they began to prepare for the invasion. It was then that Mileena discovered the piece she needed to fit the puzzle of Kitana's treachery.

A sorcerer guarding the Portal had seen the princess enter the Portal with the human Liu Kang. Kang also happened to be the largest threat to Kahn. She immediately went to Kahn with this information. However, before she reached Kahn, Kitana arrived and killed her, as well as the sorcerer that saw too much.
Mileena was resurrected by Shao Kahn himself. She found that Kitana had been accused of high treason by the High Courts of the Outworld and was now running for her life from Reptile and Jade. Mileena was needed by Kahn in the coming battle, not only for her ability to read her sister's thoughts, but also because of her savage fighting style that could only benefit him in the battle with the Chosen Warriors.
After Sheeva left Sindel's side as bodyguard to finish what was started with Motaro, Mileena was appointed her protectorate. But when things started to fall apart, she sought out Baraka and made plans with him to flee the doomed fortress. However, during her escape she found an unsightly, yet familiar fan protruding from her chest.

After her death at the hands of her "sister," Mileena was reborn in the Netherealm, only to discover that she had a new oppressor, Shinnok. As a slave, she faced the destruction of her very soul if she refuses to fight by his side. However, she saw that the coming battle posed ample opportunity to leave the damned asylum of the corrupted. As she walked onto the battlefield, she saw an all-too-familiar face, her own. Kitana had picked up her fans and donned her ninja costume once again to fight for the freedom of Edenia.
As she battle raged before her, Mileena smiled to herself in the darkness. Vengeance, however pedestrian, was always an enjoyable experience, and few ever have the chance to avenge their own death. Ensuring that if Shinnok fell, her survival would be intact, she goes into the war with both eyes open.
Eternally conniving and deceitful, she will see to it personally that her motives are fulfilled.
