If you have received a formal invitation and will attend our wedding reception, we appreciate that you select from the dinner menu below. So that we can keep track of everyone’s meal selections, please send a separate form for each guest over the age 13. (We will be providing special meals for our younger guests.) If you decide to change your selection at a later time, please let us know by February 14, 2005. Kindly include any special dietary requests in the form below and we will do our best to accommodate your needs. Thank You.
