I understand and agree to these conditions for participation.

1. USA Dance, Inc. its officers or volunteers cannot be held liable for injury sustained by persons attending this event.
2. Persons attending this event do so at their own risk.
3. No responsibility for loss of articles left in the ballroom or event facility can be accepted by USA Dance, Inc. or its officers or volunteers.
4. All persons attending this event shall be bound by the USA Dance, Inc. rules for competitive events.
5. Please give us your USA Dance ID#, printed name and signature and return with entry form.
