Thank you for joining the USA Dance Inc. Your annual membership dues entitle you to a bi-monthly newsletter from the national organization, a quarterly newsletter from the local chapter, reduced admission to many of our dances, registration priority and reduced fees for workshops, as well as dance information listing other dances in Colorado and the greater Denver area.

PLEASE NOTE: You are required to join the national organization to be a member of the local chapter. Local dues are $12 per year. The amounts listed below are the amount for national dues and should be submitted to Gerald Bonmer.

Please complete two (2) copies of this form and send one (1) copy with a check in the amount or $12.00 made payable to USA Dance Inc.- Rocky Mountain CO Chapter #5006 to:
