Monitoring Your Condition
The following is a basic chart to track your symptoms, pain levels, sleep, etc. You may use it as a guide or template to make your own charts.
I have also included a text version to make it easier for you to copy and paste it into your own word processing program (like MS Word, Excel, Word Perfect or even Word Pad and Notepad), and make the changes you need.
There is also a printable version. For this to be printable, the font needed to be quite small, otherwise it would take pages to print out on.
