This will work best for those who have easy access to a photocopier. Save at least one blank mylar so that you can copy it. I copy mine onto plain white paper. Be very sure you get it in the copier straight. Now you can take that plain paper copy and mark it up with a black marker. Use a good dark one. I use what we call a "sharpie" but any kind should work. I find that I'm ever so much faster with a marker than with the ol' Rapidograph pen. If you make a mistake, oh well, you only waste a sheet of plain paper, and sometimes you can use the scissors to cut out the little mistakes as well.
Now this white paper is not going to go thru the reader, no way. I went to the local office supply store and got a box of clear mylar transparency sheets for photocopiers. They came in a box of 100, so I think I'm set for a while. Just photocopy the marked paper sheet onto the blank mylar. The photocopier seems to fill in very well and I've had no trouble with the reader seeing it.
One other little thing I found I had to do when using these sheets - I don't punch the holes at the top of the sheet. I couldn't find a paper punch that small anyway. I made a couple of marks on my sheet cover to indicate where the border of the card is, near the top and near the bottom. Then when I insert my card, I just line up the marks and tape the card to the holder with clear tape at the top of the card. This way, if your photocopier got it the least bit crooked, it can be corrected.
