Design Considerations Page
I used all the form elements, text boxes, radio, checkboxes, dropdown option list, and a comment area, so that I could get used to programming them. Of course making the basic mailto: form was easy but I still have to find a way to get all the information entered to be included in a form. That was the hardest part of the whole thing.
The Script I have for the first Splash screen that will hopefully redirect you to the actual form page was taken from www.javascripts.com because they have hundred's of free JavaScript codes available to the web developer. Although many of their codes are more for fun, some actually are quite good.
