Here's an explanation of how I attempted to disambiguate the fire Subtype. As written in the Glossary of the 3.5e Monster Manual, the fire Subtype contains creatures which are immune to fire–based attacks and vulnerable to cold–based attacks; it opposes the cold Subtype, which contains creatures which are immune to cold–based attacks and vulnerable to fire–based attacks.
The problem with this is that the fire Subtype must also contain elementals and outsiders with a connection to the Elemental Plane of Fire, in order to be consistent with the air, earth, and water Subtypes. There may be partial, but not total, overlap; two separate Subtypes are needed.
Resolution: in the Xaeyruudh campaign, the fire Subtype contains elementals and outsiders with a connection to the Elemental Plane of Fire; the new warm Subtype opposes the cool Subtype and contains creatures which are immune to fire–based attacks and vulnerable to cold–based attacks.
