Form 2208 U.S. Department of Labor Naturalization Service No. 98543 United States of America Declaration of Intention Invalid for all purposes seven years after the date hereof State of Pennsylvania Eastern District of Pennsylvania In teh District Court of the United States I, Joseph Zadrozny, aged 32 years, occupation carpenter, do declare on oath that my personal description is: Color white, complexion Ruddy, height 5 feet, 0 inches, weight 150 pounds, color of hair Black grey mix, color of eyes hazel, other visible distinctive marks, none I was born in Wies Wielodius Poland on the 15th day of March anno Domini 1876; I now reside at 3119 Mercer Street, Philadelphia, PA. I emigrated to the United States of America from Hamberg, Germany on the vessel Unknown; my last foreign was Wies Wieldoius, Poland; I am yes married; the name of my wife is Rosie; she was born at Poland and now resides with me. It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign prince, potentate, state, or severeignty, and particularly to the Republic of Poland, of who I am now a subject; I arried at the port of New York in the State of New York on or about the 26th day of May, anno Domini 1900; I am not an anarchist; I am not a polgamist nor a believer in the practice of polygamy; and it is my intention in good faith to become a citizen of the United States of America and to permanently reside therein: So help me God.
