The NANP includes all countries listed in the left frame on this page, the country's name is a link to the country's page on my website except for the United States since I no longer keep a listing for that country.
Concerning the NANP, I have found two excellent sites which are the most accurate and up-to-date I could find.
Here are a list of links you could find interesting:
List of area codes and cities: This includes all area codes in the NANP, to find a city or area code rapidly, the best thing to do would be to load the page and then use your browsers' ability to "Find in Frame" found in the "Edit" toolbar, enter your city or area code and click "Find Next", you should have your result promptly (note: some browsers do not have this function).
Locator Guide: Same thing than previously, except that it is listed by area code instead of states/provinces/countries.
NANP map: Excellent map showing all current and future area codes in the U.S. and Canada.

This page is also quite interesting and complete, but is a lot heavier in graphics and java applets, so if you have a slow connection, it may not be recommanded.
