This small insert came with a "Head Like A Hole" Cd, with a sticker on it that says "made in sweden" .
The front lists all the bands under 'TVT Records', all the bands under 'TVT Soundtrax', all the bands under 'Wax Trax', and all the bands under 'Blunt Recordings'.
The back has places to fill out your name and address, so you can be "get our new catalog, and get on our mailing list".
