            Case History:    
            CC: 58 year old F presents  [1]   pain and discomfort of both eyes over the past month  [2]   Referred by PA-C for "drop in"
             appt for painful eye and decreased vision.  The last recorded eye visit was 07/25/2004 with the original surgeon, but her chart isn't available to me at this visit.  She is s/p
            PKP, OU x 8 yrs with RGP wear after that. It is unclear why she had the
            PKP. She denies rigid contact lens wear prior to the surgery.   1. Entry sc  OD 20/40-  OS 20/60-      2. SLE -  Both corneas have a PKP button with both having a haze. Staining
            OU. No cells or flare in AC either eye. RGP is riding low OU  3. I'm not sure what this is. What do you think?    What
            Dr. Hom Says:  After some time, I did receive a eventual
            diagnosis from the corneal consultant. This is stromal herpes
            keratitis which cleared completely after a month of medical therapy.
