This form is designed as a way for me to get to know you better. The information submitted to me will allow me to tailor our session to meet your needs. When contacting me please keep the following in mind:

1. Personal contact is preferable and calls are welcome at any time.
2. Gift amounts are only discussed on the phone, not by e-mail.
3. Appointments may be scheduled for any hour on any day, but 24-hour notice is helpful although not always necessary.
4. Messages may be left on my phone or cell phone. No suggestive language will be tolerated, of course.
5. Out-call is available and in-call for a SELECT few. To be considered for in-call, the following information is required, either on the form below or on the phone.
