JUDICIAL NOTICE REQUIRED (West's Ann.Cal.Evid. Code (2002), §§ 451, 453, 459).
1. NOTICE IS HEREBY GIVEN that a FTB Disclosure Officer is presumed to know the information set out below. In reasonable reliance upon the same information, I DEMAND complete, open, and honest disclosure of the information I have requested in the attached Request.
