JUDICIAL NOTICE REQUIRED (West's Ann.Cal.Evid.Code (2001), §§ 451, 453, 459).
1. I request, that the OFFICE OF THE SECRETARY OF STATE prepare, and deliver to me by mail at the address set out above, exact photocopies of the public record(s) required to be filed with and maintained by the Secretary of State:
(a) Within 70 days after the date of commencement of its legal existence, the governing body of each public agency shall file with the Secretary of State on a form prescribed by the Secretary of State and also with the county clerk of each county in which the public agency maintains an office, a statement of the following facts:2. My authority for making this request is set out below:
1. The full, legal name of the public agency.
2. The official mailing address of the governing body of the public agency.
3. The name and residence or business address of each member of the governing body of the public agency.
4. The name, title, and residence or business address of the chairman, president, or other presiding officer, and clerk or secretary of the governing body of the public agency.
(b) Within 10 days after any change in the facts required to be stated... an amended statement ... shall be filed as provided therein....
(c) It shall be the duty of the Secretary of State and of the county clerk of each county to establish and maintain an indexed "Roster of Public Agencies," ... which shall contain all information filed as required in subdivisions (a) and (b), which roster is hereby declared to be a public record.

In enacting this chapter, the Legislature, mindful of the right of individuals to privacy, finds and declares that access to information concerning the conduct of the people's business is a fundamental and necessary right of every person in this state.3. I temporarily waive and ultimately reserve my Right to actual physical inspection of said record(s). I enclose my check in the amount "not to exceed $10.00" to cover the costs, not to exceed $0.10 per page or a statutory fee to be disclosed to me, of copying and mailing of said record(s). If you anticipate exceeding that amount, please stop and deliver such record(s) as you have developed to that point.
West's Ann.Cal.Gov. Code (2001), § 6250.(a) Public records are open to inspection at all times during the office hours of the state or local agency and every person has a right to inspect any public record, except as hereafter provided....
(b) ...each state or local agency, upon request for a copy of records that reasonably describes an identifiable record or records, shall make the records promptly available to any person upon payment of fees covering the direct costs of duplication, or a statutory fee if applicable. Upon request, an exact copy shall be provided unless impracticable to do so.
(c) Each agency, upon request for a copy of records, shall within 10 days from receipt of the request, determine whether the request, in whole or in part, seeks copies of disclosable public records in the possession of the agency and shall promptly notify the person making the request of the determination and the reasons therefor....
(d) Nothing in this chapter shall be construed to permit an agency to delay or obstruct the inspection or copying of public records. The notification of denial of any request for records required by Section 6255 shall set forth the names and titles or positions or each person responsible for the denial.

4. I request photocopies of the current, valid Government Code 53051 filings for the following entities:
a. Franchise Tax Board [and/or] FRANCHISE TAX BOARD [and/or] STATE OF CALIFORNIA, FRANCHISE TAX BOARD.
b. Superior Court of California, In and For the County of Nevada [and/or] SUPERIOR COURT OF CALIFORNIA, COUNTY OF NEVADA [and/or] NEVADA COUNTY SUPERIOR COURT.
c. Nevada County District Attorney [and/or] NEVADA COUNTY DISTRICT ATTORNEY'S OFFICE [and/or] DISTRICT ATTORNEY'S OFFICE OF NEVADA COUNTY [and/or] OFFICE OF THE NEVADA COUNTY DISTRICT ATTORNEY [and/or] OFFICE OF THE DISTRICT ATTORNEY, COUNTY OF NEVADA.
d. California Highway Patrol [and/or] CALIFORNIA HIGHWAY PATROL.
e. Nevada County Sheriff [and/or] NEVADA COUNTY SHERIFF'S OFFICE [and/or] OFFICE OF THE NEVADA COUNTY SHERIFF [and/or] COUNTY OF NEVADA, OFFICE OF THE SHERIFF.
4. For each record(s) which has not been filed for each entity, please complete and deliver to me an official Certificate stating that fact.
I certify under the laws of the State of California that the foregoing is true and correct and that this is my signature appended below.
