| ALL PLAYERS - IMPORTANT MATCH NOTICE All players are to sign the card 15 minutes before kick off. To allow for the card to be handed in with the match ball and officials fee before kick off. Failure to sign will mean the said player will not play. The game will not be allowed to commence until the completed card has been received by the game official. |
