 This page is the core of the Third Option Website. The layout of all pages have been kept as simple and direct as possible to reduce loading times.  I have also used the electronic form format for all surveys and questionnaires.  This format is invaluable in preserving the anonymity of the respondents which is absolutely essential.  Please insure this anonymity by not using your name or the true names of other in your comments.  Further, due to the nature of the questions and the need for your absolute candor please complete the surveys when you are alone and free from possible interruption.
