Consent Form

To be signed by all participants (any participant under the age of 17 will need a signature of a parent or guardian). No entries can be accepted without this signed form. Please read the following very carefully. Please be sure you fully understand before signing.

Notwithstanding anything to the contrary contained herein, it is understood that each entrant by the act of exhibition waives all claims against the day’s committee, and/or any of the members, officers and agents, volunteers therefore (hereinafter called the committee) for any and all injury, loss or damage which the entrant may sustain or incur as a result of or arising out of the operations of the said committee.

The entrant, his or her heirs, successors, administrators or assigns, further agree to indemnify the said committee by reason of any claim whatsoever, made by any entrants helpers, employers, employees, executors, administrators, successors or assigns.

The entrant further agrees that the entrant’s member, officer or agents shall be the sole risk and responsibility of the said entrant.

The entrant (or guardian where applicable) acknowledges that he/she has read and perfectly understands the above document and is in full agreement with its contents.

This document shall be binding on the entrant, his/hers heirs successors, administrators, executors and assigns.

Thereby waive all claims against the Western Newfoundland Equestrian Club Horse Show for any and all injury which may result.

I fully understand that the decision of the committee is final and agree to abide by the decision made by the committee. I agree to accept responsibility for the action of myself and horse(s) at all times.
