Safety Sign-Ins are in case you should have an accident while on a club ride, the other member will know how you want things handled.  I cover myself by providing my info to someone else while on club rides.

Please print out (preferably duplex) the form below, complete it as fully as possible, and get it back to me.  You can either mail it ahead of time or provide it to me prior to your ride with the club.  I also use it update my personal member database and cross reference it with the HSTA Blue Book for accuracy.

I do not give this information out, but keep it for use in the event that you are incapacitated and need someone to handle matters.  All information is voluntary and the more complete the information you provide, the better I can handle your emergency.  I do not pass judgment on anything you put down, and it will be treated as Confidential.

Feel free to update me with new information as much and as often as you want.  I will always carry the most current information while on club rides.  If you think something is missing on the form, please add it in.
