We have contacted a company called Creative Concepts. They are a Destination Management Team. This company is a full-service planning and coordination firm specializing in corporate incentive programs, special events and meetings. They can easily provide services for the largest convention to the most intimate party. From inception to completion, they creatively and professionally orchestrate each event, with your specific goals in mind.

Among their areas of expertise are planning, site selection, theme parties, decorating, catering, transportation, on-site management, VIP dinner and show reservations and a wide variety of entertainment. Additionally, they maintain a complete roster of talented and experienced models, product demonstrators, interpreters, and narrators to suit every occasion.

They have requested the information contained in this questionnaire to help them better serve us.

Once we have gathered the information requested here, we will then submit it to Creative Concepts to see if their services will be financially feasible for our group.

If not, the information obtained will still be helpful for us to plan the event on our own. Please take the time to seriously answer this questionnaire. The sooner we have a commitment from everyone, the sooner we can finalize the plans for next year's party. And the sooner we do that, the cheaper it will be for everyone.
