Health Program
Management Leadership and Employee Involvement
Management Commitment - The facility's
management will be actively visible in the improvement and administration of
the safety and health program.
Management will take an active part in the safety committee meetings,
and significant safety and health issues will be discussed at their weekly
management meeting, including all near misses and any recordable
incidents. Goals and objectives will be
established for safety and health issues.
Safety and health is integrated into the overall planning system at the
facility. Input from employees will be
gathered for key issues by an annual employee survey, a bi-annual attitude
survey, and during the quarterly all-employee retreats. The information gathered will be combined
with data from the audit program and other management concerns to formulate the
site's annual performance plan. The
annual plan will establish employee's specific goals and objectives.

Planning - Safety and health will be part
of the planning process for changes in equipment, materials, and
processes. A management change process
that includes employee participation will be incorporated into the safety and
health program. Employees actively participating
in the process, such as the selection of equipment, will lead to the purchase
of better equipment than would have been procured otherwise. Weekly management meetings will be scheduled
to discuss and coordinate any significant safety related items. All incident investigations will be
discussed with planned action items assigned and tracked through to
completion. Supervisors will meet with
all employees each day in a pre-shift meeting to discuss any safety related
items.

Written Safety and Health Program - All
critical elements (Management Leadership and Employee Involvement, Worksite
Analysis, Hazard Prevention and Control, and Safety and Health Training) and
sub-elements of a basic safety and health program will be included in our
site's written program. Our written program
will be updated annually or more often if needed, and will be required reading
and implemented by all employees.

Top Management Leadership - The
general manager will be the head management representative and will take an
active role in safety and health issues, their discussion, and resolution. All managers will set proper examples
through their use of appropriate PPE, procedures, etc. Contractor and temporary employees will also
be covered by the same level of protection through the safety and health
program. Contractors Injury and Illness
Rates will be reviewed prior to awarding any contracts.
Documents necessary to implement this element:
Employee Involvement - Employees will be
meaningfully involved in the site's safety and health program, such as serving
on the safety committees, submitting safety suggestions, identifying hazards,
reporting near misses, performing job safety analysis (JSAs), participating in
accident investigations, and delivering training to other employees. Employees selected on various committees
will be selected by other employees. Safety
and health responsibilities will be part of all employees performance
requirements, (management and employees) and they will be held accountable for
their individual performance. All
employees will be informed and instructed regarding our complaint procedures
and who to notify if unsafe conditions are observed. The hazard will be investigated immediately and tracked until
abatement is completed and the employee notified of the corrective action.
A written system for employees' safety and health concerns
has been developed. These concerns can
be brought to management's attention in numerous ways. Employees can tell their immediate
supervisor, a safety committee member, the Safety Coordinator, or present it at
the pre-shift meeting.

Joint Labor-Management Committee - Our
safety committees were formed in 19??.
There will be a safety committee on each of the three shifts. The membership of the committee is purely
voluntary, anyone who is interested can join, and there is no limit to
attendance. Management, including the
Plant Manager, the Human Resources Director, the Plant Safety Director, and two
additional Plant Management Staff, will have a strong presence on the
committees. The committees will meet at
least once every three weeks and conduct walk-through audits once a month on
shifts other than their own. Persons on
safety committees will receive additional safety and health training to assist
in their hazard recognition abilities as well as accident investigation
procedures. Recordable and minor
accidents will be discussed during the safety committee meetings for possible
corrective action. The Safety Manager
and the Human Resources Manager will facilitate the meetings and take minutes,
however, employees are free to make comments, raise issues, and discuss
possible solutions during the meetings.
Employees are free to either make suggestions, or ask their safety
committee representative to raise issues, and get hazards corrected in a timely
manner. The strong management presence
on the committees demonstrates the level of commitment at the facility and will
not hinder employee input.

Contract Workers - Our
safety and health program includes a comprehensive contractor program, which
requires pre-contract-review of past safety and health performance, contract
provisions regarding safety and health performance, and provisions for
cancellation of the contract. All
contract employees are required to receive site specific orientation training
prior to working on the site and must comply with company safety rules and
procedures. Also, this program includes
provisions to ensure that all contractor tools used on the site comply with
safety standards and manufacturer recommendations.

Authority and Resources -
Management will commit to safety and health protection by providing adequate
resource allocations. Resources include
a full-time safety training coordinator and a part-time onsite nurse, EMT, or
other First Aid Person. Approximately
____ of the ____ employees participate as volunteers in safety committees. The 19?? Budget for safety was $_______________. In addition, another $___________ was used
for Process Improvement Teams (HIT Projects).
A number of outside resources will be used to provide safety and health
expertise and outside audits, including insurance companies, private
consultants, and 21 (d) consultation.
Appropriate safety responsibilities will be vested in a number of
individuals, including the Plant Manager, Supervisors, Safety Coordinator, and
individual employees. All employees
have the authority to shut down the production lines at any time for safety
related concerns.

Line Accountability - All employees at the
facility will have some level of accountability for safety. Managers' and Supervisors' overall performance
will be tied in part to accomplishment of safety related objectives. All hourly employees will have a safety
element built into their performance reviews.
A positive disciplinary process is in place and will also be sued for
safety related actions. The system is
designed to avoid disciplinary problems by encouraging the proper behavior and
performance in a proactive approach.
Our discipline procedures (four step) will be enforced in a fair and
consistent manner for all managers and employees. The current incentive program is one that rewards employees for
participation on the safety committees, and a variety of safety related
activities will be rewarded by the issuance of "safety bonus
bucks". The specific performance
based incentives will not be used as disincentives for reporting accidents.

Safety and Health Program Evaluation -
Different organizations will be used to provide outside audits of the safety
and health program. A private
consultant will conduct an annual facility audit. The ____________ Insurance Company conducts a quarterly loss
control audit, and the ________________ conducts annual facility audit. Each organization conducting audits will
assist our managers and employees in evaluating our total safety and health
program as well as provide recommendations for improvements for specific
elements.

Worksite Analysis - Every month, the Safety Committee are
responsible for conducting site inspections.
All employees are expected to perform inspections of their own work area
prior to commencing any job. Employees
will be involved in the worksite analysis of their specific job. Baseline safety and health surveys will be
conducted for all jobs where potential hazard exist and update survey will be
conducted whenever changes are made to processes or equipment, or an accident
occurs. Nationally recognized
procedures will be used for all surveys,
New processes, materials and/or equipment will be analyzed
prior to start-up. A team consisting of
the Plant Engineer, Maintenance Supervisor, Safety Coordinator, and two Safety
Committee members will review and determine potential hazards. A job hazard analysis will be performed, and
a checklist is developed to prevent or control potential hazards, for each job
as well as any change in process or equipment.
Several methods will be used to analyze trends with
accidents. The OSHA 200/300 log will be
analyzed by area, type, and severity.
Accidents will also be graphed by year and type, and location of all
injuries will be mapped on a floor plan, and a description of the event is
posted.

Hazard Prevention and Control - Managers have the authority
and responsibility to hire outside certified safety and health professionals.
Supervisors conduct accident investigations on all
recordable incidents. The Safety
Training Manager reviews all accident investigations to assure that they are
thorough. All investigations are
reviewed and discussed by the safety committee and also at the weekly managers
meeting.
General safety rules are written and included in the
employee handbook. PPE, such as safety
glasses with side shields, hearing protectors, and safety shoes are provided
free of charge and mandated for use in the plant.
A preventative maintenance (PM) schedule will be included in
our computer system. The Maintenance
Supervisor will be designated as the person responsible for overseeing this
program. Adequate schedules for PM
activities will be established. Also
included on this system are hours worked, safety incidences, inventory list,
and JHA/JSA. A tracking system of work
orders and other scheduling will be developed.
The facility contracts with an occupational health nurse or
EMT. The nurse/EMT will do follow-up on
employees injuries and is available to address injuries when onsite. The health nurse also evaluates injuries and
makes referrals to appropriate physicians who specialize in occupational
health.
The following safety and health programs are developed and
will be followed by employees:
All employees will review and sign off that they read and
understand their role in these programs.
Managers are responsible to ensure tat these programs are
implemented. Managers are responsible
to ensure that these programs are implemented when conditions warrant. All employees will be accountable for
following these programs.
A baseline survey will be conducted on all processes where
hazardous chemicals are used. Any
changes in these processes will require a new survey be conducted. Results will be maintained for each survey
conducted.

Safety and Health Training - The
company will have a full-time position for safety training at this
facility. The employees and management
will receive comprehensive safety and health training that addresses hazards at
this facility. All employees will
attend six (6) hours of safety orientation prior to starting their jobs in
production or administration. Employees
then receive on-the-job training on a one-to-one format with an experienced
operator for at least a week before they are left alone at a production
station. All safety and health
committee members will receive an additional 10-hour OSHA course for further
training in hazard recognition as well as understanding of OSHA. Supervisors need to receive additional
training in accident investigation that encourages them to look for causal
factors for an accident and not to find fault with employees.
