Documents required for H1B Visa Transfer

1) OF- 156 FORM click here for PDF format
or write a request letter to the above address with a self envelope stamped
2) PASSPORT ORIGINAL
3) I-94 attached and pinned to passport
4) EMPOLYER PETITION ORIGINAL or WORK PERMIT COPY ( our H1 original papers )
5) letter from present employer ( click here for Sample letter ) requesting the department to transfer your H1B Visa.
6) MONEY ORDER FOR 100$ + 45$ = 145$ (should take it separately for $100 and $45 ) in favor of UNITED STATES DEPARTMENT OF STATES get it from postal department they will charge only 85cents where as bank charges $5.
7) SELF ADDRESSED ENVOLOP with 33cents stamp , in case if they any thing from you.
send the above documents by UPS express or other services with a return postage paid ..cost you about 24 dollars to way .
Recent changes in fee and correspondence address
NOTE :
The reciprocity fee for India was recently changed from $100 to $75. Thus, if an Indian national applies for visa revalidation through the U.S. Department of State (DOS), the total fees for each applicant would be $120 ($45 + $75). Please note that all fees must be made by certified check,money order, or corporate check, made to the order of the "U.S. Department of State." Cash or personal checks are not accepted. In the case of multiple applicants, payment of all fees may be consolidated in one check. Moreover,
please remember that the DOS has a new mailing address for visa revalidation since earlier this year. Please note that one is for postal service and the other is for courier service:
