Send about 20 pieces of hair ½ inch long, your name,address, date of birth, occupation, brief personal/medical
history, any current concerns/questions, medications or
supplements being taken to me together with your fee.
You can sellotape a small piece of hair to the sheet which
has their details on. Address will be sent on demand.
