A few things to consider:

  • I can not personally verify every submission. I am counting on you to provide accurate information.
  • Fill out the information that relates to your submission.
  • This form will be e-mailed  to me only. The personal information provided will remain confidential and will not be posted on the web if so requested. Please include a way in which I may contact you if I need to.
  • Please submit your request far enough in advance to allow time for posting. I will post the information as soon as possible.
  • Please! Be sure to get the needed permissions prior to submitting information. Some people may not wish to have their activity or personal information posted on the web!
