The following agreement specifies what we feel is required for a safe and loving home for a bernese mountain dog.

I herein promise and agree to the following conditions:

1. To keep this dog in my personnal possession, provide proper and sufficient food, water, shelter, grooming, and humane treatment at all times.
2. To produce veterinary care at once if it becomes ill or injured along with annual vaccinations.
3. To provide this dog an ID secured to a collar or a tattoo or a micro chip for permanent identification.
4. To obey all animal control regulations in my area and to license the dog according to regulations within one month of adoption.
5. Not to sell, trade, transfer ownership, abandon, or dispose of this dog, but to notify us if custody of the dog must be relinquished. This includes release to family members.
6. Allow the rescue operation to examine the dog and its living conditions and to surrender the dog to the representative if he or she finds the conditions unsatisfactory.
7. Assume full reponsibility for the dog's actions, and for any damage done by the dog.
8. To keep this dog as my household companion. To ensure when outside unattended, the dog is in a secure fenced yard or kennel run with adequate shelter from the elements, never allowing the dog to roam or run loose.

Additional requirements may be specified at time of adoption.

The undersigned hereby agrees to abide by the terms of adoption listed above.
