Use a wire brush or wire wheel to be sure cables and the inside of the lugs are bright and shiny before crimping, or this is what will result! Note, that a week before this happened it was discovered that this connection was hot and the connection was tightened. Its obviously not enough to just keep connections tight. Once loose, oxygen can get in and oxidize the connection. Thats why some people recommend grease on connections. In any case, the cable and battery lug should have been taken apart, cleaned and then tightened.
