You have deteriorated beyond what words can describe, but you haven't failed (which would be an "F") since you're alive still.
If you've been like this a while, you should definitely be in therapy and/or on medication. If looking up a shrink and setting an appointment seems like too much, try to get a good friend to do it for you. Things are not going to just get better all at once, but it can happen - slowly.
