In the pilot episode we learn that Doug and Carol were once an item, but have broken up. (That was one factor that led to Carol's suicide attempt.) Now that Carol and Doug are back together they both seem happier. You can tell by the way that they act that Carol is the serious one and Doug is the joker.
Doug and Carol have been through many adventures together. In "Exodus" they were trapped in an elevator with a little girl who was running out of oxygen. But they worked together and managed to make it through. Carol stuck by Doug when his Dad died and Doug stuck by Carol after she was in the robbery at the convience store. They even break hospital rules together. When Doug saved that baby's life by holding him in the hospital to do an ultra rapid detoxe without the mother's permission, Carol was right there with him.
Doug are Carol are engaged now and are almost always seen together. After next season there will be no more Doug though. George Clooney has announced that he won't be coming back. He wants to produce and act in movies. The producers of E.R. have said they will leave it open so George Clooney can come back and guest star, but what will happen to Doug and Carol. Hopefully they will still be together and Doug just won't be around as much. All we can do is wait and see.
