James was a physician and surgeon trained in medicine at University of Pennsylvania, Philadelphia.  He has first attended University of North Carolina graduating in 1846.
He moved to Demopolis and began the practice of medicine.

Dr. Ruffin was a Mason and a member of the Knights of Pythias.
