Parent Information Guide
                                                        


DISCIPLINE POLICY:
PROJECT Synergy Works students are expected to follow the rules in the Pontiac School District Student Handbook and any other site-specific rules that are established by the PROJECT Synergy Works program or the Site Coordinator.

In order to maintain a safe and fun environment, we will work closely with your child to ensure proper behavior.  If, however, the problem  persists, a Discipline Referral report will be written.  You and the school principal will receive a copy of the report.  Consquences are as follows:
     1st Referral: Comference with child, parent, Site Coordinator and referring staff member.
     2nd Referral Conference with child, parent, Site Coordinator and referring staff member.
     3rd Referral: Suspension from PROJECT Synergy Works Program.

Depending upon the severity of the behavior problem, steps may be skipped.



HEALTH INFORMATION:
Illness Policy:  If a PROJECT Synergy Works participant becomes ill, a parent or autorized person will be called to pick up the child.

Emergency/Health Information: Parents are asked to provide emergency and health information on the enrollment form.  Information on the form includes home and places of employment phone numbers, names and  phone numbers of family physicians, health  problems the child has, if any, and directions as to handling emergency child illness and injuries.  It is important that we have this information and that it is current at all times.

Medication: If your child requires medication during PROJECT Synergy Works hours, an authorized form must be completed.  If your child receives medication during the school day, a separete form must be completed for PROJECT Synergy Works.  The site coordinator will be albe to provide you with the proper form.

Only trained staff will give students medication.  All medication must be kept in the original container.  Information regarding dose and time of administration must be completed.

FIELD TRIPS:
PROJECT Synergy Works offers field trips and/or special activities to our participants.  Some of these activities may require additional money and/or transportation.  Information regarding upcoming field trips will be available at least one week prior to the trip.

PARENT INVOLVMENT:
Parent/guardian participation is strongly encouraged at the program.  You are welcome to visit at any time, just register at the office when you come.

We would be happy to have you volunteer in the program.  Volunteers are needed to plan and chaperon special events, to provide enrichment, and to offer assistance to staff members.

CONSENT TO RELEASE INFORMATION:
The PROJECT Synergy Works program works closely with your child's school site.  As part of our program, we will be sharing information with Pontiac School District,, teachers, other school staff, contracting agencies, program evaluators and funders.  Sharing information ensures continued funding as well as the best possible services and support for your child.

Your signature onthe registration form gives us permission to access records and the exchange information regarding your child.

MEDIA RELEASE:
PROJECT Synergy Works staff may photograph, videotape and/or audiotape members and others  participating inthe program.  Any or all of these forms of media may bu used at the discretion of PROJECT Synergy Works for working with and  promoting the  program.  Your signature on the registration form in that section gives us that persmission.
