How to order:

1. Verify price of medications you are ordering with price system.

2. Please sign and mail the forms required to process you order, Release Form* , Patient History Form* and Order Form* and your original prescriptions.

3. The Canadian pharmacy will contact you to confirm your order.

4. Shipping cost are $15.00 US funds per order, not per prescription. You are we encourage you to order a 90 day supply to save you money on shipping costs. New customer orders may take up to four weeks being for delivery. Due to necessary paper work to establish your account, refills will take only a few days to ship.

5. Just print and complete these forms.
Make checks/money orders payable to McKnights Pharmacy.
