Determine the Root Cause

This is a critical stage in the definition of a solution. It is probable that any well defined solution should work. People and organizations do not intentionally design solution to fail. However, many do fail. Generally it is because the real roots of the problems were not addressed and the root cause reoccurs. It may have been assumed that the application solution, the new organization, the new leader, or the new player would keep the problem from reoccurring.

Capabilities

The major reason why solutions fail is seldom in design. It is generally something(s) in the culture or environment that is the root cause of the failure or diminished success of the solution.

Root Cause Analysis
This analysis is necessary to determine whether the intended solution or part of the solution is addressing the root cause or merely a symptom. Unless the actual cause is identified, there is no way of knowing whether the new solution will be a cure or a bandage. There are not an unlimited number of root causes, nor are they industry constrainned. The root causes are found in five catagories.
Theory of Constraints
This is a very effective way of addressing any constraints not just the root cause. The tactics of this methodology are to determine the constraints and their impact or influence. Once this priority is establish, subject everything else to its solution. Although this is not always the best way to implement solutions. It can result in local suboptimization if the domain being analyzed does not extend to the customer. It is appropriate for defining the cause of the constraint and its impact at any domain size.
