| This is the, Personal Identification form, and is used to document statistical information, for all the persons, in the family. It also includes the, Case File Number, but gives no place for the information of the investigator. So get names and all relevant information about these people |
| This is the Safety Factor form, and is used to describe any conditions in the family that may place the children at risk. Take care, to note, any supporting information, that may be listed, in the extreme, left hand collumn, as it will give details that claims to justify the other collums. |
| While... social workers, investigators and Agency staff may tell you that these forms are confidential it might be worth while, to demand a copy, of them anyway, in order to find out, what these people may be claiming about you - and/or - any part, of your family. You could use the leverage of co-operation against them, by telling them that you may agree to fill out their releases, of information, if they release a copy, of these documents, to you... |
