| I have an information sheet hanging on my tack board on the side of fridge. I decided to make this sheet when I had children so that anyone who was at my home watching my kids would have any information they needed at their fingertips. I went one step further by adding information like regularly taken medication just in case our family needed help and paramedics were called. Here is what I put on our family information sheet: - Last Name - Address - Home Phone Number - Each family member's name, birthday, and any telephone numbers (home, work, cell) - Emergency contact information, their relationship and their proximity (Ex. - Mrs. Smith, 410-555-2366, Mr. Smith's mom, Bowie, MD) - Emergency numbers (hospital, police, poison control, etc) - Family doctor's names, addresses and phone numbers - Any medical problems (illnesses, pregnancies, allergies, conditions like diabetes, regular taken medication, etc) - List of all animals, their names, their types, their ages and their vet's name, address and phone (vaccination dates esp. rabies wouldn't hurt either) |
