The geriatic care manager is a professional who is experienced in determining and meeting the needs of seniors living independantly.
The first step: is an in home care assessment in your home to evaluate your living situation and determine what services are needed to improve the quality of life and keep you safe.
This assessment may include any or all of the following areas.
*Health*
*Safety*
*Medical equipment needs*
*Emotional ststus & invironment.
Following this assessment the geriatic care manager will help you select an in-home care agency and will assist you and or your family in developing a care plan and attendant schedule.
Home health care consists of a wide variety of services to individuals who do not need nursing home care but do require some assistance in meeting their health care needs.
Services fall into two general categories,
*Skilled care & Supportive Services*.
Skilled care is given under the general direction's of a physician and consists of health services provided by licensed professionals, such as:
*Registered nurses & Professional therapists*.
Supported services are those provided as an adjunct to skilled care that enable a person to continue independant living within the home.
The in-home care agency can assist you in performing a wide variety of activities required for daily living.
Household management assistance can include:
*Housekeeper recruiting*
*Letter writing*
*Bill paying*
*Reading mail*
*Making appointments*
*Performing light household chores*
*Meal preparation*
Companion Services*
