Additional Application Information

The first page requests basic student information. It is also the place where the student will hand-write a 350-500 word essay on a topic of their choice.

The yellow guidance form requests information from the student's counselor. This information includes the GPA of the student and test scores.

The teacher recommendation form is the location for teacher input and comments.
