Below is a sample questionnaire similar to one that can be tailored to and attached below your submission for feedback. The questionnaire is divided into two parts:

ADMINISTRATIVE DATA- is used to identify and reply to individuals who visit this site and provide feedback to our members. In general, this data is only of use to The Reader's Perspective. If the visitor has a web site, we would like to check it out. Because you may wish to as well, that portion of administrative data will be provided to you.
 
QUESTIONNAIRE- is where you get feedback from readers as they respond to questions specific to your work.
