Be sure to include as much information as possible. Your Team Name, captain's name and a phone number &/or e-mail address where people can contact you about the fundraiser the Name of the fundraiser. We've added a new form that allows you to enter the information right on your computer, print it out & mail it to Kathie Smith at the Barker Village Hall.
