Accounting - Personal Financial Statement
Here is a sample of some information you may have to compile and have on hand when applying for any type of loan, or to present to your equity/debt investors.  Some people include this in their Business Plan as well.  It is important to have a Good Credit Rating, and it doesn't hurt to obtain a copy of it for attachment as well.  Showing up prepared is makes for a good impression.
You will need to have:
Name/Address/Phone # /Business Name,Address & Ph# / Social Security # / Your Registered Agent (lawyer/accountant) and their address & ph#
Income Statement
Any income left over after expenses have been paid??  Put them in your savings account.  This increases your Cash Holdings which in turn increases your Net Worth.

Do you have a Will? If so, name of Executor.

Are you obligated to pay Alimony, Child Support or Seperate Maintenance Payments?  If so, describe.
Are any assets pledged other than as desribed on schedules?  If so, describe.
Income Tax settled through (date)
Are you a defendant in any suits or legal actions?
Personal Bank Accounts carried at:
Have you ever been declared
Bankrupt? If so, describe.
