Chief Compaint: Too much damn studying to do
History of Present Illness:
Patient is a 23 year old Taiwanese male who complains of too much stress from studying
for his second year of med school and the USLME Step 1. Has complained of this for
over a year and a half after leaving UC Berkeley. Also complains of memory loss and
insomnia. Risk factors include being a med student, eating too many potatoes, and
lots of Simpsons.
Medications:
Zantac 150mg PRN
Vitamin C 1000mg/day
Centrum 1/day
Allergies:
Keanu Reeves acting, Tara Lipinski
Past Medical/Surgical History:
Duodenal Ulcer in 1991
Frontal Lobotomy 1993, 1994, 1998 (planned)
Social History:
Patient was born in San Francisco, California and has lived there for his entire
life, with the exception of occasional trips to Taiwan. Patient currently resides
in Los Angeles, CA during the school year but is frequently found at home relaxing
whenever possible in the SF bay area. Previous to attending UCLA Med, was at UC Berkeley
for four years. Enjoys backpacking, hiking, camping and other outdoor sports. Denies
alcohol, recreational drug use, or tobacco.
Physical Exam:
Patient is generally a very well nourished and in some acute distress. Looks younger
than the stated age. Some obvious mental psychosis present.
