Server
Just Before Mass
The server gets the cross from the sacristy and goes to the vestibule (the back of the church) with the priest and the lector(s), and proceeds when the choir starts singing.
Procession and Opening Hymn
When the procession reaches the altar, the server moves to the right, and the lector(s) to the left of the priest. The priest genuflects, and the lector(s) do the same; the server bows her/his head. The server brings the cross back to the sacristy, the lector(s) follow to their seat(s) at the left side of the sanctuary; the priest enters the sanctuary last, going to kiss the altar and then proceeding to his chair. The server comes back from the sacristy and goes to her/his seat at the right side of the sanctuary; the server bows to the altar when (s)he crosses behind it.

The server gets into place immediately after the Gloria, and opens the Sacramentary (which has been set to the appropriate page before Mass) for the priest to say the Opening Prayer.

Announcements
During the announcements, the server crosses to the other side of the sanctuary (remembering to bow to the altar). (S)he brings the chalice, water cruet, and any extra patens/ciboria with hosts (not the extra chalices) from the credence table (in the sanctuary) to the right corner of the altar.
Presentation of the Gifts
Those bringing up the gifts do so immediately after they receive the collection from the ushers. The person carrying the money should precede the two others carrying the paten of hosts and flagon of wine. The person with the money should present her/his gift to the priest first, followed by the other two. After the group gives the gifts to the priest, the priest bows, and those bringing up the gifts bow back, in unison. Then they return to their seats.
The server goes with the priest to receive the gifts. The collection should be put in the sacristy.
Preparation of the Altar
After putting the collection in the sacristy, the server gets the lavabo bowl (silver bowl), water pitcher, and lavabo towel (distinct from purificators - no red cross) from the credence table. After the priest has lifted both the paten and the chalice, asked God to bless both the bread and wine, and when he is bowing to say a silent prayer, the server moves toward the right corner of the altar. (S)he washes the priest's hands and allows him to dry his hands on the lavabo towel, which should be resting on one arm. When the priest returns the lavabo towel to the arm, the server then returns to her/his seat at the left side of the sanctuary.
Prayer over the Gifts
Eucharistic Prayer
During the Eucharistic Prayer, the server kneels and rings the bells (three short, distinct rings) when the priest elevates the Host, and again when the priest lifts the Chalice.
Lord's Prayer (Our Father...)
Sign of Peace
During the Sign of Peace the server brings the (two) extra chalices, with the accompanying purificators, to the right side of the altar. The server DOES NOT pour the Precious Blood into these chalices; the priest does.
Holy Communion
After (and only after) the priest has given himself Holy Communion under both species, he will give Holy Communion to the server and EMEs. They may remain kneeling, but they customarily come and stand abreast in front of their kneelers to receive Holy Communion.
After Communion, the server clears everything from the altar, then crosses back to her/his original seat at the right side of the sanctuary, remembering to bow towards the altar when crossing behind it.
Period of Silence
The server opens the Sacramentary (which has been set to the appropriate page before mass) and sits attentively, prepared to go to the priest for the Prayer after Communion.
Prayer after Communion
When the priest stands and says "Let us pray", the server comes to the priest (on his left), opens the Sacramentary to appropriate page, and positions it for the priest to read the Prayer after Communion. After the priest has completely finished this prayer, the serve returns to her/his seat. (S)he puts down the book but remains standing for the Final Blessing.

Recessional and Closing Hymn
The server gets the cross from the sacristy immediately after the blessing and then goes to the foot of the steps, and stands facing the tabernacle. After the priest kisses the altar, when he comes around to the front of the altar and genuflects toward the tabernacle, the server bows her/his head, then turns around (always to the right, i.e. clockwise) and processes out. If there are lectors remaining in the sanctuary, they come down to the foot of the steps and join the server. The server stands to the right, and the lector(s) to the left, as in the opening procession. The priest genuflects, the lectors(s) genuflect with him, then turn and process out behind the server but before the priest.
When the server reaches the back of the church (the vestibule), (s)he turns and faces the priest, holding the cross aloft. The lectors, if recessing, move to the left and right of the server and face the cross. The priest should bow to the altar and recite a brief prayer/exhortation. The lectors bow in unison with the priest.
After Mass
The server puts everything away, locks the tabernacle (returning the tabernacle key in the sacristy, turns off the sound system and the lights, and closes both doors to the sacristy (making sure that they are locked).
