Probably the most well-known and widely available Mayhem bootleg, but has since been made obsolete (it was primarily interesting when the Morbid material wasn't available on CD). Note that there is an earlier version of this bootleg on Land of the Rising Sun Records--as that's another Bullmetal label, the Warhammer version is more of a repress than a knock-off.
Tracks 5 and 8 are the only studio tracks Mayhem did with Dead, from the 1990 promo. These have appeared on the "Projections of a Stained Mind" compilation and on several other bootlegs.

Several versions of this bootleg exist, all containing the Sarpsborg gig. On vinyl, there is the original LP and a picture disc. On CD, there is the original jewel-case pressing, a digipak version, a later jewel-case pressing, and a pressing on the "Anti-Grishnackh" label which includes the "Pure Fucking Armageddon" demo. While they all use the picture of Dead, the various versions of the bootleg mostly use a different size, color, or font for the band logo and the title, so they're not too hard to tell apart. The digipak CD and subsequent jewel-case CD pressings are the only versions of the bootleg with the Lillehammer tracks. The one I own is the second jewel-case pressing, which in my opinion has the nicest cover in terms of picture quality and font/logo choice, but has a cheap looking back insert.
The Sarpsborg gig is, I believe, Dead's infamous first live show. The Lillehammer gig is supposedly the band's first ever live show--some b/w video footage of the band from '86 exists, which I believe is from this concert.
I'm sort of baffled why so many people (people who should probably exit metal immediately!) complain about the sound quality of the Lillehammer tracks--from what they say, one would expect the cover songs to sound like industrial machinery with vocals over the top rather than anything recognizeable. While the recording quality is indeed bad and the vocals and drums are the most audible part of the mix, nothing is unlistenable or unrecognizeable to me, especially considering the classics they're covering. I've heard hundreds of recordings (including supposed "studio" recordings) that sound worse than this!
"Danse Macabre" is only a minute or so long excerpt (like Frost themselves used to do at live gigs), not a full cover.
Note that "Danse Macabre" and "Black Metal" aren't listed as being together on the back insert. I know the digipak CD lists them as seperate track indexes, but as I don't have that version, I'm not sure if they're actually indexed seperately on that disc. Here, they're together in a single track.

Update! There is a new digipak version of this bootleg. Cover looks to be a b/w picture picture of a castle or house. There are apparently also 6 bonus tracks, 2 '90 tracks with Dead (I'm guessing "Carnage"/"The Freezing Moon") and 4 '86 instrumental tracks (probably from the demo). I do not have it yet so all info is tentative.
Vocalless rehearsal of most of the "De Mysteriis..." album. In my opinion, this is actually infinitely better than the released album, which was ruined by the pathetic operatic vocals. The bass is also much more prominent than on the final album recording.
Ironically, "From the Dark Past" is missing here, which would make this CD a complete rehearsal of the "De Mysteriis..." album. Note that this song is included on the "In Memorium" bootleg, which appears to have tracks from this same rehearsal session.
Note that there are two different versions of this bootleg; the only difference is the printing on the discs. One has Mayhem's logo, the label number, and title printed in white on a plain black disc. The other version of the disc is printed with a picture of the band along with all the aforementioned information.

Warning: I've been informed a CD-R bootleg of this CD has surfaced with a professionally printed disc and inserts. The CD-R version actually has 16 tracks, with the vocalless rehearsal version of "Pagan Fears" being included between tracks 8 and 9 above. Since the extra track is listed on the CD-R version, the tracklisting is the easiest way to tell the CD-R from the original. The inserts are supposedly more red-tinted than the those with the original CD so that the white portions of the cover are slightly reddish. Also, any original CD should have "In Memorium" written in the matrix on the underside of the CD in the inner band where the CD pressing information goes.
Note while it does have a similar cover and uses the same back cover photo, this CD has nothing to do material-wise with the bootleg 7"s of the same name.
Tracks 1-7 are from what is probably the best-quality rehearsal with Dead that exists. These tracks were officially released on the "Out From The Dark" LP, which is where the bootleg was probably sourced from.
Track 8 is the infamous "All the little flowers..." sing-along which appears elsewhere. It is listed on the bootleg as "Promo '90," although it supposedly first appeared on the original version of the "Deathcrush" demo.
Tracks 9-13 are a vocalless rehearsal of "De Mysteriis..." songs, likely the same recordings as on the "From The Darkest Past" bootleg. However, the vocalless version of "From The Dark Past" appears to be exclusive to this bootleg.
Tracks 14-15 feature bass and vocals by Culto Culus/Occultus of Perdition Hearse fame (after Dead's death, he was briefly in the band), and appear to be the only recordings done with him that have been released (or even exist). Although they're of a bad quality, they're listenable, but not too interesting, as he just tries to sound like Dead.
Despite its shortcomings, the quality/variety of the material here make it the best of the Mayhem bootleg CDs.

A bootleg of the band's 1986 demo. Apparently a more recent repress on the "Century Blast" label exists, which is essentially the same except for a different print on the disc itself and a slightly changed layout to reflect the different label. Popular stupid opinion and trading lore seems to indicate that the studio (Tracks 1-5) and unmixed (6-9) portions once compromised seperate demos "Pure Fucking Armageddon" and "Voice of a Tortured Skull" tapes. The original "PFA" tape always had all 9 tracks--read Metal Forces #19 for proof.
Along with the intros, tracks 2, 3, and 5 are the only songs with any sort of audible "vocals." But they're only muffled grunting. I've heard that Euronymous and Necrobutcher handled vocal duties in the earliest years of Mayhem, but it's not indicated who exactly is "singing" here.
Track 4 is unrecognizeable except for the Lay Down Your Soul... post-chorus instrumental pause.
Track 6 is just an intro mostly made up of insane screaming and drumming.
Surprisingly, the unmixed tracks are less muffled and the guitar is more up-front.
The material here is certainly interesting, and I would say even better to some extent than "Deathcrush," but the lack of vocals and relatively bad sound-quality make this more of a novelty CD than anything else.

CD comes in a simple gatefold sleeve (like a digipak without the plastic tray). Most of this bootleg's appeal is historical, as this was the first live show played by the "reformation" lineup after Euronymous' death. Partial footage of this show was released officially as the "Live in Bischofswerda" video.
Okay sound quality. Most of the song introductions and stage banter sound clear, but during the songs themselves, the vocals tend to get buried under everything else. Interesting only for the novelty/historical factor, or for tremendous fans of the "Wolf's Lair Abyss" mCD. This is a decent but relatively boring attempt by a band who at the time fooled themselves into thinking they could match the standard of their earlier stuff.

Since the tracklisting on the bootleg is so partial and error-filled, I'm not even going to bother listing the incorrect tracklisting. Zemial is not mentioned anywhere on the bootleg. Note that a tape version of this bootleg without the Zemial tracks apparently exists.
Thou Shalt Suffer is of course the Ihsahn/Samoth/Ildjarn death metal project; since the release of this bootleg, all of the Thou Shalt Suffer material that appears here has been officially rereleased on CD.
Track 5-8 are the same as on other bootlegs. I know the studio tracks with Dead were part of a 1990 promo, and I guess that might have included the "Deathcrush" version of "Necrolust," which is why they're often together on bootlegs.
Tracks 11-13 are from the original press of Zemial's "Sleeping Under Tartarus" EP. Note that both versions of the "For the Glory of UR" CD only have two of these tracks--as far as I know, "Falling Into the Absu" is only available on CD on this bootleg.
Most interesting for the 1990 Mayhem rehearsal and the complete Zemial EP. Admittedly, a much better bootleg when the Thou Shalt Suffer stuff was unreleased.

No track indexing numbers are given on the back insert, just a listing of the various parts. Note that there's really nothing here that isn't unreleased. I assume when the bootleg refers to the "original uncut version" of the "Deathcrush," they mean the "All the little flowers..." track (track 9 here, also available on several other Mayhem bootlegs) is included as it was supposedly part of the original demo. If you want to get really, really nitpicky, it sounds to me like this version might have a split-second of extra material at the very beginning, where you hear what I guess is Maniac screaming. Otherwise, it's virtually the same as the officially-released "Deathcrush" CD, just a tiny bit lower in sound-quality.
Miscellaneous
There is apparently a bootleg called "A Warlord's Swansong" floating around, though as it's extremely limited (to 100?), it is most likely a CD-R. The bootleg supposedly contains an Attila-era rehearsal, a '95 rehearsal, and some earlier material taken from other bootlegs.

Despite having a profesionally-printed disc and inserts, this is indeed a CD-R. This is really just a compilation of material taken from other bootlegs.
Tracks 1-6 are from the "Ha-Elm Zalag" bootleg LP. I've also seen the material mentioned as being from 1990, but I'm not sure which date is correct. I haven't heard how the tracks sound on the original vinyl, but they are of almost unlistenable quality here, as the instruments are mostly just a wall of sound. Track 6 is the same unreleased track Emperor incorporated part of in "Ye Entrancemperium."
Tracks 7-8 are the two studio tracks with Dead and track 9 is the "All the little flowers..." sing-along, seen elsewhere.
Tracks 10-16 are the same Dead rehearsal from the "In Memorium" CD and the official "Out From the Dark" LP.
Tracks 17-19 are the first 3 tracks from the "War & Sodomy" bootleg LP. These sound like they're sourced from mp3, as the beginning and end of each song seem to have artifacts and compression distortion. The overall sound quality is pretty good, although this seems to be one of the rare recordings where the instruments actually overpower Dead's vocals to some extent.
Not too desirable being a CD-R, and even less so due to the overlap with some other bootlegs. While it is nice to have parts of the bootleg LPs on digital format, they also represent the worst sounding material here.
