BIG NEWS!!!! After 20 years of solo practice in Sun Lakes, Dr. Boles has merged his solo medical practice with Dr. Robert Bloomberg (and Dr. Yao)

Dr. Boles is spending some time in the Phase One office at 25237 S. Sun Lakes Blvd, Suite 3, and also some office hours in the main office at 6301 S. McClintock, Tempe, 85283 (See schedule below) The phone number for scheduling will remain the same. 480-895-2010. Dr. Boles has ceased being medical director of Chandler Valley Hope drug and alcohol treatment center and their outpatient treatment centers in the state as of April 2009.

Carolyn Boles will remain in private counseling practice, and may also see patients by appointment at the Phase One offices of Dr. Bloomberg.

The Renaissance office will remain available for seeing patients by appointment only. There will not be daytime staffing of that office unless an appointment has been made to see a patient there.

Just as our office is a friendly and informal place where people drop in and deal with a variety of problems, I am hoping that the Website allows our patients, neighbors and any visitor to come to know us better and to share our philosophy. In 2009 we are celebrating our TWENTY-SECOND YEAR ANNIVERSARY in Sun Lakes.
