• Three passport size colour photographs each of husband and wife.
• Two photocopy of IC each of husband and wife. If your IC address differs from current address, enclose two photocopies of either electricity/water, house phone/mobile phone or Astro bills.
• Two photocopy of marriage certificate and each child birth certificate. 
• Original plus two photocopies of latest Income Tax Form J each of husband and wife.
• Total annual household income must exceed RM36,000.00 and RM60,000.00 for Indonesia and Philippine maids respectively.
• Original plus two photocopies of last 3-month payslips each of  husband and wife.
• If Self-employed-sole proprietor, enclose two photocopies of  Borang B and D from ROB 
• If Self-employed company director, enclose two photocopies of  Form 9 and Form 49 from ROC.
• If Employed, enclose original company letter plus two photocopies  (written in Bahasa Malaysia) stating job position and monthly income of husband and wife. The letter should be addressed to:
