Review of Important Program Policies
/ Student Contract
Instructions:
Read the following carefully and print it out when you have
finished. Be sure to sign the bottom of the form after it is printed. Use the
copier to make a copy of it for your notebook. Then, turn in the original copy to your
instructor. When you have completed this "Program Policies" section, YOU
WILL HAVE TO TAKE A TEST ON IT!! TAKE THIS SERIOUSLY!! If you fail the test, YOU
WILL HAVE TO TAKE IT UNTIL YOU PASS!
1. I understand that I must always read the board
each day to check for project due dates and other information on make-up work, tests,
quizzes etc.
2. I understand that I must report to class as soon as my
bus arrives.
3. I understand that I am not allowed to bring a Walkman
or beeper/pager into class. The school does not permit them in class. If I am
carrying one, it must be put IMMEDIATELY into a drawer or my locker. If not,
they may be confiscated and sent to the office.
4. I understand that I can NOT bring food from outside of school into
the classroom. I may sometimes be permitted to have food from the supermarket or
school store. However, I should not eat in class until permission is given by
my instructor, especially near the computers or other electrical equipment.
I should NEVER come into the classroom with food or beverages, and begin eating in
class! This will result in discipline points and a possible office referral.
5. I understand that I must get an admit slip BEFORE reporting
to class anytime I am absent or late. The school will now be doing attendance electronically on
the computer. If I am not INSIDE THE CLASSROOM when
attendance is taken, I understand that I will be marked absent, and that there is no way
this can be changed electronically by the teacher.Therefore, I would
have to get an admit slip and will be marked late. I also will receive discipline points
for each unexcused late I accumulate.
I MUST present any note for absence to the Attendance Secretary PERSONALLY, BEFORE
CLASS! Notes are NOT to be given to the teacher.
It is my responsibility to ask for any assignments or notes from the day before.
If it is an unexcused absence, I will receive two zeros for my daily grade for that day.
However, I am still responsible for all worksheets, quizzes, tests, projects or any other
assignments I missed due to absence.
6. I understand that I must fill out a work
report (Click HERE for illustration) EVERYDAY.
If I fail to do that five times in one marking period, there will be a percentage taken
off of my Attitude grade.
7. I understand that if I am absent more than five
unexcused days in a marking period, I will lose a percentage off of both my Lab Quantity
and Attitude grade(Click HERE for a look
at Grading Procedures) for the marking period.
8. I understand that I can NOT go into the Printing classroom at
any time, without first asking permission and taking a pass. The only exception is to
visit my locker before class has begun.
9. I understand that unless I have completed all of my assignments
and asked my teacher if he has additional work for me to do. I should not leave my seat or
distract any other student.
10. I understand that I am not to use any equipment or computer
without first asking the teachers permission.This includes walking into the
classroom and working at the computers. I MUST GET THE TEACHER'S PERMISSION
FIRST!!
11. I understand that I must have a pencil or pen and my
notebook here everyday. I must also remain alert and never put my head
down during lecture or class instruction.
12. I understand that I must wash the drawing surface of my desk
if requested to do so. I am responsible for pushing in the stool everyday. If,
I am in the PM session, I must put up the stool. If I am at my computer station, I
must push in the chair and return the mouse to the center of the mousepad,
straighten the monitor to face forward, and straighten the keyboard. Failure
to do this may result in failing attitude grades.
13. I understand that I must NEVER cut on a drawing table,
wooden table or drawer unit top.
14. I understand that I must keep an updated notebook to
be submitted once a marking period for a TEST GRADE.
15. I understand that I must keep all project grade sheets
and attach them to my projects. I must never throw away a project. I must
keep them together, and submit them as a portfolio for a unit test grade and a
project grade.THE PORTFOLIO IS VERY IMPORTANT, AND IS REQUIRED TO BE
HANDED IN FOR SPECIFIC COURSES!!!
16. Students MUST adhere to the following work times: AM Session:
All students will work until AT LEAST 9:50 AM. This includes ALL early dismissal schools
and post secondary students. PM Session: All students will work
until AT LEAST 12:55 PM. This includes ALL early dismissal schools and post secondary
students.(Except Monsignor Donovan, if necessary.)
17. Students leaving before their dismissal times will be
reported to the office for a cut. This includes leaving one minute early. The first
time, students may receive a written warning, HOWEVER, this policy agreement should
serve as EVERY STUDENTS' FIRST WRITTEN WARNING! Students should abide by
their school's listed dismissal time. If there is a problem with the dismissal schedule,
you will be permitted to discuss the issue with the Vice Principal. He is the only person
permitted to change the dismissal schedule. All students should ask for a copy of their
dismissal time.
*** NOTE: STUDENTS SHOULD NOT STAND BY THE DOOR AT
DISMISSAL TIME. BEGINNING THIS YEAR, YOU MUST REMAIN BY YOUR DRAWING TABLE UNTIL THE BELL
RINGS!!! Any student who does not follow this rule will receive a written warning, and
if it happens a second time will be awarded discipline points.***
18. ANY STUDENT WITH THEIR HEAD DOWN ON THE DESK OR AT THE
COMPUTER WILL RECEIVE ZEROS FOR THE DAY IN ATTITUDE AND LAB QUANTITY. If you do
not feel well, you may ask to visit the nurse. Continued problems in this area will result
in a disciplinary referral, and a parent conference, even for post secondary students.
19. Any unauthorized printing of material on the computer (NOT
related to the program projects or assignments) will result in zeros for the day, as well
as a charge of 10 cents per page. This
includes the copier as well.
20. I understand that there is a new
Classroom Discipline Form that will be used to administer points directly
from the teacher WITHOUT me being sent to the office. (Click HERE and review the copy of the new form). This will be
used for minor disciplinary infractions such as profanity, sleeping in class, etc. THESE
POINTS WILL BECOME RECORDED IN THE OFFICE AS PART OF MY TOTAL POINTS ACCUMULATED AND
OFFICIAL STUDENT RECORD.
21. I understand that there is NO SMOKING on any part
of this campus and property. There is NO SMOKING AREA in any part of the
building or property. I can NOT even smoke in my car on this property.
This is cause for disciplinary referral, points, suspension, and even removal from the
program.
22. I understand that there are NO HATS allowed at this
school, and that if I wear a hat, I MUST REMOVE IT IMMEDIATELY UPON LEAVING THE
BUS OR MY CAR AND BEFORE ENTERING THE BUILDING.
23. I understand that leaving the classroom early constitutes a
"cut". Three cuts are grounds for dismissal from this school.
24. I understand that there is NO DRIVING to this school, except for
post secondary students who obtain permission and a valid parking permit for the current
school year.
25. I understand that I can not ride in someone else's car to
this school unless it is with a one day permit for a field trip, or I am a post
secondary student over 18 years of age, riding with another post secondary student.
26. There may be limited access to the
Internet this year. This is due to the unauthorized downloading of certain programs
onto the classroom workstations. Students have also abused the use of checking private
e-mail and going online or "chatting", when they are supposed to be working on
specific assignments. THIS WILL NO LONGER BE ALLOWED. Students will receive a written
warning and disciplinary points. Any repeated abuse of this policy will result in THE
STUDENT BEING REMOVED FROM THE PROGRAM.
27. I understand that refusal to follow any of the above policies is
considered insubordination and could be cause for a written referral to the
vice-principal.
By signing this contract, I state that I have read ALL of these policies, and
promise to abide by them. I further understand that the breaking of these rules may be
grounds for disciplinary action and possible removal from the school or program.
