Key caveats to remember when designing your form.
- Addresses Always leave room for several addresses.
- Request the applicant indicate ALL States resided in for the past seven years.
- When requesting education data - make sure you close the loop on graduation and getting the degree. Dates of attendance are important.
- Leave room for at leat 5 employers. Request: the supervisor name; senior manager name; one peer name, from each employer. Otherwise, if the supervisor has left you will be sent to human resource records and then you will find out little.
- Many candidates have had some form of self-employment. It is important that you require them to list professional references, telephone numbers, and the relationship with the client. Often candidates use self-employment to hide unsatisfactory performance. You will need a way to weed out the offenders.
- Make a clear and unambiguous place to know whether you can contact the current employer. Make the candidate write: "yes" or "no".
- Date of birth, sex and race are vital to make sure you do not get false positives on the criminal and credit information. If your firm's attorney's are hung-up on DOB's, sex and race, I suggest you prepare an addendum to your employment application which says: The following information is required to perform your pre-employment records check and then include the release and whatever other information you intent to make your standard policy for the company.
