• Functional Performance
  Performance rating, management acceptance, achievement of goals and targets, level of supervision needed.

• Operational Disciplines
  Following rules and regulations, timeliness, environmental stress, care of equipment, integrity, reliability.

• Inter-Personal Disciplines
  Ability to cooperate with others (peers, outside contacts, customers), potential for growth.

This sheet is an excellent check list for hiring authorities to use while referencing. It reminds them to cover all the topics with each reference contacted.
