REGISTRATION PROCEDURES

Reservations

1)
Reservations made by phone or email will only be held for 72 HOURS pending receipt of your paperwork.

  • When submitting your team registration form, be sure to include your
    correct tournament fee.
  • It is not necessary to send a copy of your roster when registering.
    Completed original Roster/Release forms are due prior to your first game.

Registration

1) Print down the "Team Registration" form and mail with payment.

  • a) Each team registration form must list the primary team contact (coach or
    manager) with all contact information.

    b) Organizations which enter multiple teams are required to list the primary contact
    for each of the teams, not the general program coordinator on each form.

2) Print down the "Roster/Waiver" Form.

3) The completed "original" roster/waiver form must be returned prior to your first
game to tournament headquarters.

  • a) Teams which use multiple forms to record parent releases must turn in all
    forms in the following manner:

4) All additional sheets (with the team and coach's name listed on each) must be
attached to an official team roster listing all players rostered and turned in as a
complete package.

5) Teams which bring the completed form to the event are required to send a copy
of their form to the contact numbers listed above 72 hours prior to the event.
