Who may participate? Children and adults of any age who would benefit from warm-water swimming or exercise may swim at the Easter Seal Aquatic Center. Included are individuals who are chronically or temporarily physically limited, those with a physician's referral, persons age 60 and above, and children participating in our swim lesson program. All applicants must meet the requirements of the Participant Eligibility Policy. |

Application Procedures:
1. Easter Seals requires a complete registration application, a Health History Questionnaire and an attending physician's medical form. All forms must be completed before an intake can be scheduled and use of the facility can begin. 2. Also included in your packet is a Participant Eligibility Policy. Please read this policy carefully. All applicants must sign the participant form acknowledging the Participant Eligibility Policy. 3. Completed forms should be mailed or brought to the aquatic center along with the $8.00 processing fee. This fee covers paperwork processing as well as the initial orientation and evaluation appointment with and Easter Seals Instructor. if forms are mailed, call the pool one week after mailing to make sure they have arrived. 4. Persons needing assistance with dressing or in the water will need to bring someone (spouse, friend, caregiver) with them each time they come to swim. This person should also be present at the orientation appointment. There is no charge for the person providing assistance. 5. Applications will be kept on file at the aquatic center office. It is the swimmer's responsibility to keep information current. If you have a change in medications, physical condition, or you have moved, please have your file updated. 6. Original aaplications will be valid for two (2) years from competion date. All swimmers are required to file new medical forms every two years.
